Abstract: 

Introduction: The pathway that a psychic patient go through in seeking PSYCHIATRIC services and the way that he or she refer to a general practitioner or traditional healers is the main aim of this study. Method & Material: In this descriptive study, a total number of 600 patients who refer to psychiatrists or Ebn-e-Sina PSYCHIATRIC HOSPITAL were randomly selected and a questionnaire was completed for each of them. Results: Results showed that, the first choice of 27% of the patients is psychiatrists, 19.7% of them is general practitioners, 11.2% other specialists and 42.2% of them is traditional healers. Based on the results, 50.7% of all cases have risited a traditional healer during their treatment 44% of the cases were guided to a PSYCHIATRIC medical center by a relative or a friend and traditional healers had no role in such referrals. Conclusion: The findings clear that in Hormozgan province, traditional healers have still a great role in attracting psychic patients. This problem not only escalates treatment expenses, but also causes a delay in receiving medical services.

Abstract: 

Background: More PSYCHIATRIC visits, especially non-emergency ones, to emergency DEPARTMENTs (EDs) of general HOSPITALs have been observed in recent years. The aim of this study was to determine the characteristics of PSYCHIATRIC visits to the ED of Rasoul-e-Akram HOSPITAL, Tehran, Iran.Methods: In this cross-sectional study, during a two-month period, all PSYCHIATRIC presentations and consultations to the ED of the studied HOSPITAL were included. The required data were gathered by psychiatry chief residents and were documented in pre-designed checklists.Results: About 0.01% of all patients presenting to the ED needed the PSYCHIATRIC visits. Men consisted 50% of the total patients with mean (±SD) age of 36.41 (±14.7) years. About 51% of them had the indication of the emergency PSYCHIATRIC visit while 47% had the indication of HOSPITALization in the PSYCHIATRIC ward. Non-emergency visits were not related to demographic characteristic, previous PSYCHIATRIC disorders, substance abuse and physical diseases.Conclusions: Non-emergency visits take a high percentage of PSYCHIATRIC visits in ED and regarding limited sources for PSYCHIATRIC emergencies and Long visiting time, this percentage can hinder the process of giving services to real emergency PSYCHIATRIC patients.

Abstract: 

BACKGROUND: PSYCHIATRIC team plays an important role in treating and taking care of PSYCHIATRIC patients. Using this team's experience seems necessary for enhancing care services. The aim of this study was to assess PSYCHIATRIC team's experience of working in different PSYCHIATRIC groups.METHODS: This is a qualitative study with phenomenological approach. Samples were chosen using purposive sampling method. Samples included 10 personnel from PSYCHIATRIC wards of 4 HOSPITALs in Isfahan city in 2007. Data was gathered using deep interviews which were recorded on tapes. Data was analyzed using Colaizzi’s seven steps method.RESULTS: Results from participants’ experiences were summarized in 3 main concepts: “communication”, “ability” and “conflict”; and 5 sub concepts: “cooperation”, “self acknowledgement”, “hope”, “qualification” and “multiple roles”.CONCLUSIONS: Results of this study could be used in future programming to enhance care services for PSYCHIATRIC patients in HOSPITALs.

Abstract: 

Objectives: The aim of this study was to pilot implementation of the code of conduct for the children and adolescents in a child and adolescent PSYCHIATRIC center.Method: The present quasi-experimental and prospective study was carried out in child PSYCHIATRIC ward and clinic of Imam Hossien HOSPITAL, Tehran, Iran, in two six-month phases. The first phase included the research executive-educational strategy and during the second phase the code of conduct for the children and adolescents was implemented in the PSYCHIATRIC ward and clinic. The subjects were parents of children referred to child psychiatry clinic and ward. The used questionnaire was extracted from the code of conduct for the children/ adolescents and completed by the parents. Pretest questionnaires were filled out in the first phase and prior to any intervention, and posttest questionnaires after pilot implementation of the guideline. The number of pretest (and posttest) subjects in the ward and clinic were 33 (24) and 60 (56), respectively. The data was analyzed using Chi-square and dependent and independent t-tests.Results: There was significant difference between pretest and posttest scores of the subjects referred to PSYCHIATRIC ward in receiving instructive and informative booklets (p<0.001), but there was no change in the level of satisfaction for the physical conditions and facilities of the ward. In the clinic, the level of satisfaction of clients with the staffs communication behavior (p<0.01), and with the clinic conditions and facilities (p<0.001) increased in the posttest compared to the pretest.Conclusion: Some simple changes such as improving the physical conditions and facilities, and information services could increase the clients’ satisfaction. Nonetheless, changes like modifying the staff behavior, and their interaction and communication with the clients require more time, training, practice, and rehearsal.

Abstract: 

Introduction: Aggression is the second most common complaint in the PSYCHIATRIC emergency DEPARTMENT. The aim of this study was to determine the frequency of various PSYCHIATRIC disorders among aggressive patients, which could be useful in their treatment process. Methods: The present study is a descriptive and cross-sectional study conducted in 2023. The sample was selected through census. 1027 patients with various PSYCHIATRIC diagnoses were identified, and finally 294 were eligible for inclusion in the study. After identifying the patients, their medical records were reviewed and based on that information, the checklist was completed. Results: 66 (22. 4%) of the patients with bipolar I disorder presented in the mania phase as manic. The most common mental illnesses identified in men and women were: 41 (32. 3%), 25 (15%) with bipolar I disorder, manic phase, and 32 (25. 1%), 25 (15%) with bipolar I disorder, manic phase. 38 (25%) of patients under 38 years of age had bipolar I disorder, manic phase. There was also a significant relationship between age (P=0. 008) and gender (P=0. 049) with PSYCHIATRIC disorders (P<0. 05). Conclusion: Aggression, as a worrying behavior for the general health of the community, can often be accompanied by PSYCHIATRIC disorders. Bipolar disorder, especially in the manic phase (manic or non-manic), has the highest frequency among aggressive patients, which can be related to gender and age.

Abstract: 

Introduction: The aim of this study was to determine the reasons for ordering the PSYCHIATRIC consultation for the inpatients of a teaching general HOSPITAL in Tehran, and evaluating the demographic information and type of diagnosis. Method: In 3 years' time, 300 inpatients (174 females) who were sequentially referred for a PSYCHIATRIC consultation from the twelve wards of Hazrat Rasoul HOSPITAL in Tehran, were assessed by a demographic questionnaire and PSYCHIATRIC interview. Results: About 40% of referred patients aged 18-40 years. Statistical analysis revealed that the internal medicine ward had the highest referral rate (44%). Two most common reasons for referral were general PSYCHIATRIC assessment (13%) and evaluation of patients' psychotropic medications usage (11%). The most common diagnoses were mood disorders (24.4%) and coping with an illness (13.7%). Conclusion: PSYCHIATRIC problems were common in the different wards of this general HOSPITAL.

Abstract: 

Introduction: Many medical diseases and their related signs and symptoms are not identified in routine assessments in emergency DEPARTMENTs. Objective: We investigated the prevalence of abnormal findings in physical examination of the patients who were newly admitted to emergency DEPARTMENT (ED) of a PSYCHIATRIC HOSPITAL. Methods: We studied 200 patients (143 males, 71. 5%) who were recently admitted to the ED of a PSYCHIATRIC university HOSPITAL during a 4-month period in 2018. A thorough physical and neurological examination was performed on each subject. Results: Nearly all of the patients (99. 3%) had at least one positive finding in physical (non-neurological) examination and 95% had at least one positive neurological finding. We also found at least one problem in gastrointestinal, respiratory or cardiac system of 22. 1%, 24% and 33% of the subjects, respectively. The most frequent findings were in cranial nerve I (30%, more common in substance abuse disorder), and visual acuity (52. 6%), as well as recent memory (%68. 7), attention (%33. 7), orientation to time (%29. 5), hypokinetic movements (%28. 1, more common in females and psychotic patients), akathisia (41. 6%), and skin (%85. 1, more common in substance abuse and personality disorders). T wave changes were seen in 31. 9% and arrhythmia in 16% of the patients. Conclusions: Abnormal findings in examination of the patients admitted to EDs are very prevalent, but most of the abnormalities are not detected or reported in the routine ward examinations. More emphasis should be placed on the examination of olfactory nerve (especially in patients with substance abuse disorder), cognition (especially in older adults), extrapyramidal system, heart, abdomen and skin (especially in patients with personality disorder) during the first week of admission in a PSYCHIATRIC emergency setting.
